Probiotics and prebiotics: fermented foods and intestinal health

Probiotics, prebiotics, fermented foods: behind these terms lies one of the most promising scientific advances in modern nutrition.

Since the discovery of the gut-brain axis and the sequencing of the human microbiome, our understanding of the role of diet in gut health has undergone a revolution. Kefir, sauerkraut, kimchi, miso, kombucha: these traditional foods that have been used for thousands of years in many cultures are now finding rigorous scientific validation. Understanding the distinction between probiotics and prebiotics, knowing which foods are the best sources and knowing the documented benefits is now a health prevention skill in its own right.

1. Probiotics vs prebiotics: the essential distinction

These two terms are frequently confused or used interchangeably. However, they represent very different realities:

The image that best illustrates the probiotic-prebiotic relationship: if the microbiota is a garden, the probiotics are new seeds that I plant, and the prebiotics are the fertilizer that nourishes the plants already in place. The two are complementary.

## 2. Fermented foods: natural sources of probiotics

Fermented foods are produced by the action of microorganisms (bacteria, yeasts, molds) which transform the sugars and proteins of foods, enriching their nutritional profile and generating beneficial living microorganisms:

## 3. Prebiotic foods: feeding the good bacteria

Prebiotics are specific dietary fibers that selectively feed beneficial bacteria in the microbiota. The best food sources are:

  • Garlic and onion: very rich in fructo-oligosaccharides (FOS) and inulin. These are the most powerfully prebiotic foods available in common kitchens. Their regular consumption is associated with a significant increase in Bifidobacterium.
  • Leek and asparagus: excellent sources of inulin. Asparagus is one of the foods richest in prebiotics relative to its caloric content.
  • Jerusalem artichoke: champion of prebiotics with an inulin content of up to 20% of its fresh weight. To be introduced gradually to avoid bloating.
  • Green and slightly ripe banana: rich in resistant starch, a type of prebiotic fiber which escapes digestion and ferments in the colon. The riper the banana, the more the resistant starch is transformed into simple sugars.
  • Legumes: their soluble fibers (galacto-oligosaccharides in particular) are among the best prebiotic sources, in addition to their protein and iron contributions.
  • Whole grains: oats (beta-glucan), barley, rye. The soluble fibers of oats are particularly well documented for their prebiotic effect and their action on cholesterol.

4. Scientifically documented benefits

Research in microbiology and gastroenterology has documented specific benefits for certain strains and certain indications. Here are the best established associations:

  • Diarrhea associated with antibiotics: this is the most solidly documented indication. Lactobacillus rhamnosus GG and Saccharomyces boulardii significantly reduce the duration and severity of diarrhea during antibiotic treatment, according to Cochrane meta-analyses.
  • Irritable bowel syndrome (IBS): several strains of probiotics (notably Bifidobacterium infantis and Lactobacillus plantarum) reduce the symptoms of IBS in randomized clinical trials.
  • Microbiota diversity after a fermented diet: a study published in Cell (2021) by Justin Sonnenburg's team showed that a diet rich in fermented foods increased the diversity of the microbiota more effectively than a diet rich in fiber alone in healthy adults.
  • Mental health and the gut-brain axis: a meta-analysis published in General Psychiatry (2019) showed that probiotic supplementation reduced depressive symptoms. The gut produces 90% of the body's serotonin.
  • Immunity: modulation of the microbiota by probiotics and prebiotics improves the mucosal immune response and reduces the frequency and duration of respiratory infections in several studies.

5. Supplements vs foods: my clear choice

The question of probiotic supplementation comes up frequently. Here is my positioning based on the data:

  • Fermented foods have the advantage of diversity: they provide a complex cocktail of microorganisms that is difficult to reproduce in a supplement. They also provide additional nutrients (proteins, vitamins, minerals).
  • Probiotic supplements have the advantage of specificity: certain specific strains have precise clinical indications (Saccharomyces boulardii for post-antibiotic diarrhea, Lactobacillus rhamnosus GG for digestive infections). In these indications, supplementation has its place.
  • The dietary prebiotic outperforms the supplement: prebiotic foods (garlic, onion, legumes, oats) are more diversified, more bioavailable and less expensive than isolated fiber supplements.
  • My priority: maximize fermented foods in my daily diet (yogurt, kefir, raw sauerkraut, miso, tempeh) and prebiotic foods (garlic, onion, legumes), before considering any supplement.

    💡 tips to remember

    1. Probiotics = beneficial live microorganisms (yogurt, kefir, kimchi). Prebiotics = fibers that feed beneficial bacteria (garlic, onion, legumes). The two are complementary.
    2. Kefir is the most diverse fermented food in terms of microorganisms: much more than yogurt. A glass a day is an excellent gesture for the microbiota.
    3. Sauerkraut and kimchi only retain their probiotics if they are consumed raw or unpasteurized: cooking destroys living microorganisms.
    4. Garlic and onion are my best prebiotic foods: their FOS and inulin selectively nourish Bifidobacterium. A simple culinary gesture has a strong impact.
    5. During antibiotic treatment: I take a specific probiotic (Saccharomyces boulardii or Lactobacillus rhamnosus GG) - this is the best documented indication for supplemental probiotics.
    6. A diet rich in fermented foods diversifies the microbiota more effectively than a diet rich in fiber alone (Cell study 2021 - Sonnenburg).
    7. Banana Unripe is better than very ripe banana for the microbiota: the resistant starch of green banana is an excellent prebiotic, converted into simple sugars as it ripens.
