Calcium equals dairy products: this equation is so anchored in people's minds that it has become a reflex. However, it is both incomplete and reductive. While dairy products are indeed excellent sources of calcium, they are far from being the only ones. Many people - lactose intolerant, vegans, or simply those who consume little dairy - can perfectly meet their calcium needs through other food sources, provided they know the right ones. This article offers you a complete and nuanced overview of calcium: its roles, its needs, its sources and the factors that influence its absorption.

1. Calcium in the body: much more than bones

Calcium is the most abundant mineral in the human body: adults contain approximately 1,000 to 1,200 g of it, 99% of which is in bones and teeth in the form of hydroxyapatite. But this remaining 1%, circulating in the blood and tissues, plays vital biological roles:

  • Muscle contraction: calcium is the signal that triggers the contraction of muscle fibers, including the heart muscle
  • Nervous transmission: it is involved in the release of neurotransmitters at the synapses
  • Blood coagulation: several coagulation factors require calcium as a cofactor
  • Hormonal regulation: involved in the secretion of many hormones
  • Enzymatic activation: cofactor of many metabolic enzymes

Blood calcium levels are kept within a very narrow range by a sophisticated hormonal system involving parathyroid hormone (PTH), calcitonin and active vitamin D. When food intake is insufficient, the body draws on its bone reserves to maintain blood calcium levels - a survival mechanism which, repeated over years, leads to osteoporosis.

## 3. Sources of calcium: well beyond dairy products

If dairy products are rich in calcium and make it easier to cover, many other sources make it possible to achieve the recommended intakes:

4. Bioavailability: not everything is equal

The calcium content of a food does not directly reflect the amount actually absorbed by the body. The bioavailability of calcium varies considerably depending on the dietary source and accompanying factors:

  • Antinutrients reduce absorption: oxalates (spinach, beetroot, rhubarb) and phytates (unfermented wheat bran, unsoaked seeds) bind to calcium and reduce its intestinal absorption. Spinach, despite its high calcium content on paper, is not a good source because its oxalates block absorption by more than 95%.
  • Vitamin D increases absorption: it is essential for the active absorption of calcium in the small intestine. Vitamin D deficiency significantly reduces the efficiency of calcium absorption, regardless of dietary intake. Vitamin D and calcium are inseparable for bone health.
  • Gastric acidity promotes absorption: calcium is better absorbed in an acidic environment. Proton pump inhibitors (omeprazole, pantoprazole), taken long term, can reduce calcium absorption by raising gastric pH.
  • Lactose facilitates absorption: the natural sugar of milk facilitates the transport of calcium through the intestinal wall, which partly explains the good bioavailability of dairy calcium.

5. Osteoporosis: the key role of diet and lifestyle

Osteoporosis is a disease characterized by a decrease in bone mass and a degradation of the microarchitecture of bone tissue, increasing the risk of fracture. In France, it affects 1 in 3 women and 1 in 5 men over the age of 50.

Several dietary and lifestyle factors influence the risk of osteoporosis beyond calcium intake alone:

  • Vitamin D intake: essential for calcium absorption and bone mineralization. An inseparable duo.
  • Physical activity with load carrying (walking, running, weight training): mechanical constraints stimulate bone formation. It is one of the most powerful protective factors.
  • Protein intake: necessary for the synthesis of bone collagen. A protein deficiency weakens the organic matrix of bones.
  • Tobacco: reduces calcium absorption and accelerates bone loss, particularly in women.
  • Excess alcohol: disrupts bone metabolism and reduces bone formation.
  • Excess salt: increases urinary calcium excretion.
  • Excess coffee: slightly increases urinary calcium excretion, but the effect is modest and compensable by an adequate calcium diet.
