Physical activity and prevention of cardiovascular diseases

Cardiovascular diseases are the leading cause of death worldwide. And yet, up to 80% of premature heart attacks and strokes could be avoided by lifestyle changes, of which regular physical activity is the most powerful lever.

In France, cardiovascular diseases are responsible for 140,000 deaths per year, or one in four deaths. Behind this figure lie myocardial infarctions, strokes and heart failures which, for the vast majority, are not fatalities but the consequences of modifiable risk factors.

Regular physical activity acts directly on almost all of these risk factors: blood pressure, cholesterol, diabetes, obesity, chronic inflammation, vascular health. It is today considered by the international medical community as the most effective cardiovascular prevention intervention available. Here's why, and how to get the most out of it.

1. How cardiovascular diseases develop

The vast majority of cardiovascular diseases are the consequence of a progressive process called atherosclerosis: the deposition of lipid plaques (fat, cholesterol, cellular debris) inside the arteries, which thickens them, stiffens them and gradually reduces blood flow. When a plaque ruptures and forms a clot, it is a heart attack or stroke.

This process is influenced by several modifiable risk factors, on which physical activity acts in a direct and documented way:

The epidemiological data accumulated over fifty years are clear and convergent:

  • 35% reduction in the risk of coronary heart disease in physically active vs. sedentary people (Warburton et al., CMAJ 2006)
  • 25% reduction in stroke risk for people meeting WHO recommendations (Lee et al., Stroke 2003)
  • 50% reduction in cardiovascular mortality in cardiac patients who practice regular physical activity after their episode (Taylor et al., Cochrane 2014)
  • Even moderate physical activity, 150 minutes of brisk walking per week, reduces the risk of heart attack by 30 to 35%
  • Each 10 MET-hours per week of additional activity reduces cardiovascular mortality by 20% (Nocon et al., Eur J Cardiovasc Prev Rehab 2008)

4. The paradox of intense exercise: pay attention to the dose

Moderate exercise is unequivocally beneficial for the heart. Intense and regular exercise is also good for the vast majority of healthy people. But recent studies indicate a slight increased risk of arrhythmias (notably atrial fibrillation) in competitive athletes who have practiced intense endurance sports for many years.

This phenomenon, sometimes called the athlete's paradox, concerns a minority of high-level athletes and absolutely does not call into question the benefits of regular physical activity for the general population. The conclusion is simple: the optimal dose is between inactivity (the greatest risk) and extremely intense (risk marginally increased for certain arrhythmias). For the vast majority of people, moving toward more activity is always the right direction.

For people who have already experienced a cardiovascular episode (infarction, bypass surgery, stent placement), physical activity is not contraindicated, it is recommended and supervised.
